SLOAN COMFORT CARE HOME WELCOMES YOU

In 2016, a small group of individuals working in the healthcare system came together after identifying a gap in access to end-of-life care. This group saw that while most people express a desire to die at home, many were unable to return home and were forced to spend their final days living in institutions. 

 

In many communities across the country, including neighboring counties, a model known as a comfort care home had flourished, offering an alternative to those who were not able to return home, whatever the reason, and who did not wish to die in a hospital or nursing home. 

 

Accountable Health Community, Inc. was founded in 2016 as a 501c3 nonprofit with the goal to create a comfort care home in Erie County while also continuing community education around end-of-life mercy doulas and advance care planning. 

 

In 2020, the doors opened to the first and only comfort care home in Erie County, and the first guest was welcomed to the Sloan Comfort Care Home. Since then, more than 86 of our neighbors have spent their final moments in our Home.

 

We provide the home and the caregiving services, provided by a mix of paid caregivers and community volunteers. 

 

No guest or their family ever receives a bill for the care we provide, and our services are not covered by insurance plans.
